Care Coordination Services
Care Coordination services provides a comprehensive assessment of the older person’s needs and works with the individual and/or their caregiver to identify and obtain services, equipment, information, etc.
Persons who receive care coordination services are monitored through home visits and telephone contacts to determine if their needs are being met.
Eligibility Criteria :
60 years of age and older
Cost of Service :
Fees are determined on a sliding scale after deductions are given
for medical expenses. There is no fee charged for persons whose
income is at or below the poverty level.
Service Area :
Floyd, Giles, Montgomery, and Pulaski Counties; Radford City; and
the Towns of Blacksburg, Christiansburg, and Pulaski
