Transition Coordination Services
Transition Coordination Services assists individuals in moving out of institutions and back into the community. Transition Coordination is limited to 3 months prior to discharge and up to 12 months from the date of discharge.
Eligibility Criteria :
Individual of any age and/or disability that has been institutionalized for 6 consecutive months and
are eligible for Medicaid benefits upon discharge specifically qualifying for the Elderly or Disabled
with Consumer Direction Waiver (EDCD).
Cost of Service :
There is no fee for services.
Service Area :
Floyd, Giles, Montgomery, and Pulaski Counties; Radford City; and
the Towns of Blacksburg, Christiansburg, and Pulaski
