Will you age in place?

The essentials of a successful plan

The New River Valley Aging in Place Leadership Team developed the materials on this web page for individuals to form a plan to help them stay in their homes and in their communities for as long as possible.

Materials have been designed to present in a group setting with a facilitator or to be used on an individual basis. A facilitation guide is included for setting up and facilitating a group workshop.

There are five main topic areas; housing, health and wellness, transporation, personal finance, and social connections and growth, to explore in the Aging in Place Workbook along with videos that introduce the topics.

Additionally, there is an introductory video designed primarily for health and human services for professional development. There is no cost to download and use the materials for educational purposes. Your input for the use of the materials is all that we ask.

Please use the link below to complete a short questionnaire after viewing the materials and deciding how you will use them
